Patient Medical Records
Due to the retirement of Dr. Larry S. Rosenberg, Orthopaedic & Sports Specialists, PC is closing its medical practice effective November 1, 2026.
All patient medical records will be securely maintained in accordance with New Jersey regulations and applicable record-retention requirements.
How to Request Your Records
To obtain a copy of your medical records, or to request that your records be transferred to another healthcare provider, please submit a written request via email to:
Please Include the Following Information in Your Request:
- Patient full name (including any prior names)
- Date of birth
- Current mailing address
- Phone number
- Dates of treatment (if known)
- The name and address of the provider to whom records should be sent (if applicable)
For security and privacy purposes, additional identification or authorization may be required before the release of records.
Orthopaedic & Sports Specialists will continue to maintain records in accordance with all state and federal confidentiality and retention requirements.
